Transitions Mental Health Association is a nonprofit organization dedicated to eliminating stigma and promoting recovery and wellness for people with mental illness through work, housing, community and family support services.

dedicated and empathetic Case Manager to join our team at Transitions-Mental Health Association. As a Case Manager you
will play a crucial role in supporting and advocating for adults, living in our communal residential program, empowering
them to achieve their highest level of independence and well-being.
Position Summary:
Case Managers for the adult programs are expected to supervise the general health, safety, and social rehabilitation of clients. They also perform moderate facility maintenance, function as positive role models, promote cooperation and harmony in communal living, and uphold and promote the program philosophy in all duties performed. This position is full time, 40 hours a week, Monday through Friday.
Responsibilities and Duties:
1. Assist residents in all aspects of independent living including the development of social, vocational, and effective life management skills; and assisting residents with proper nutrition, cooking, shopping, budgeting, housecleaning and maintenance, and personal hygiene.
2. Recognize and be available to clients in psychiatric crisis or emotional distress and teach appropriate coping and conflict resolution skills. Provide counseling and emotional support.
3. Facilitate recreational, educational, or other program activities. This may include transporting clients, encouraging client participation, and modeling appropriate social behavior.
4. Facilitate client and/or house meetings, and assume the role of advocate in all aspects of clients’ needs and goals. This may include familiarizing clients with and encouraging the usage of community resources, support systems, services, and opportunities; and assisting clients in building healthy peer and familial support networks.
5. Promote, encourage and maintain trauma-informed practices in all services and activities. Practice and promote, both individually and in team meetings, TMHA’s Collaborative Communication Guidelines in order to facilitate clear, empathetic and person-centered communication.
6. Assist clients in determining, developing, and carrying out their own goals; review plans and goals, evaluate progress, and foster independence.
7. Maintain the cleanliness and upkeep of the residences; this may include general household chores, yard work, completion and/or reporting of needed repairs, acquisition and disposal of furniture, and assisting clients in maintaining a clean and sanitary living environment.
8. Complete program paperwork; this may include individual client files, evaluating and charting client progress, admission/ discharge paperwork, Medi-cal billing, time sheets and petty cash.
9. Act as agency liaison by consulting, collaborating, and communicating with Program Manager, SLO County Mental Health, and other treatment agencies to meet client and program objectives. This may include coordinating referrals and/or providing information to other agencies or treatment programs.
10. Attend weekly staff meetings and complete required hours of in-service training.
11. Assume other duties and responsibilities as assigned by the Program Manager.
Job Environment:
1. Moderate, physical activity and frequent driving; may include moving or lifting up to 50 lbs.
2. Involves moderate risks that require safety precautions and the risk of exposure to potentially intense situations.
Minimum Requirements:
- Bachelor’s Degree in a related Human Services field and at least 6 months social rehabilitation experience; or at least one year, full-time, social rehabilitation experience.
- Strong interpersonal skills, collaborative work style and strong oral/written and communication skills.
- Able to deal effectively with a variety of people and personality types regardless of age, ethnicity, race, religion, sexual orientation, gender identity/expression or socio-economic level.
- Ability to handle intense situations, display good judgment and decision-making skills, and remain calm and clearheaded under pressure.
- Well defined organizational and time management skills.
- Ability to work independently and flexibly.
- Must use reasonable precautions in the performance of one’s duties and adhere to all applicable safety rules and practices while acting in such a manner as to assure at all times, maximum safety to one’s self, fellow employees, clients, and other persons contacted through TMHA business.
- Must have a strong commitment to the right, ability and choice of each person to live in a safe environment; develop a meaningful role in the community; and have access to competent and continuous supports and services.
- Must have a reliable automobile and utilize own vehicle at work with company mileage reimbursement when a company vehicle is not available.
- Conditions of employment include:
Must be at least 18 years of age; possess a valid CA Driver’s License, reliable transportation, proof of auto insurance, and DMV driving record that meets TMHA’s current vehicle insurance requirements. May be required to utilize personal vehicle and auto insurance. · Attendance is an essential function of this job that requires face-to-face interaction with clients and/or other employees.· Ability to possess and maintain good physical and mental health.· Ability to work well within a team and maintain a professional, respectful attitude in the workplace.· Ability to be willing to work with supervisor oversight and direction. · Comply with a T.B. skin test or chest x-ray indicating a negative tuberculosis result and receipt of a criminal record clearance through fingerprinting. · Must be able to successfully complete First Aid and CPR training.
