Overview

Come join a team of dedicated and compassionate staff at a new 16-bed state-of-the-art Secure Residential Facility in Essex, Vermont.. You will play an essential role in the person-centered treatment of adults with severe mental illness. The facility is over 17,000 square feet with a separate full-size gymnasium. The new facility is scheduled to open in early April of 2023 and will be transitioning from the current 7-bed Secure Residential Program located in Middlesex, Vermont.

This position will provide psychiatric case work for the individuals residing at River Valley Therapeutic Residence. Duties include coordinating admissions and discharges, supporting the team with determining eligibility for admission, completing treatment plans, coordinating care with outside provider and families, and supporting residents with individual and group outings in the community. Additional duties include the exploration of financial and disability benefits, testifying in court, tracking outcomes, and overall responsibility for the coordination of care for the residents.

Mission:
The River Valley Therapeutic Residence (RVTR) aims to improve the health and well-being of Vermont's most vulnerable populations while focusing on psychiatric rehabilitation services and psychosocial treatment delivered in a recovery oriented, trauma informed, person-centered, holistic environment. The program offers a safe space to focus on recovery and community integration for those individuals no longer needing hospital level of care but who are not yet ready to transition back to the community. Telework:
During this time of transition, individuals will need to travel to Middlesex, Waterbury, and Berlin for training and shadowing. There may be some flexibility for remote work during this transition as well.

Environmental Factors

Duties are performed largely in a residential setting although trips to meet with community providers and to visit patient living situations will occur, as well as community outings with residents.

A valid driver's license is required for necessary travel. Although duties are normally performed during a regular weekday, daytime shift, specific case needs may require availability at other times.

One-to-one interaction with acutely ill and/or behaviorally dysregulated clients is involved, creating a potential risk of physical injury for the Care Manager.

Minimum Qualifications

Master's degree in a Mental Health related field AND EITHER one (1) year or more of professional experience working with individuals with serious mental illness OR a one-year field placement in a mental health clinic, psychiatric hospital, or Psychiatric Residential Program.

OR

Bachelor's degree in a Mental Health related field AND three (3) years of professional experience working with individuals with serious mental illness.

Preferred Qualifications

Licensed in mental health related field.
