Encore Village of Schaumburg is currently hiring for a part time Resident Assessment Coordinator (MDS). Will be responsible and accountable for the coordination, accuracy, and completion of the Resident Assessment Instrument and Individualized Resident Care Plan that facilitates a holistic approach to resident care.
Qualifications:
- Must be a Registered Nurse currently licensed by the State of Illinois,
- Must be a graduate from an accredited Registered-nursing program
- Must be CPR Certified
- Must have 2-3 years of long-term care nursing experience and knowledge of Medicare-PPs, MDS, and Care Planning.
- MDS certification and experience with MDS 3.0 is preferred
