Job Description:
JOB SUMMARY
The Health Information Representative responds to all requests for medical record information from various requestors including health care providers, patients, attorneys, third-party payors, disability, subpoenas, and internal requestors. All Release of Information must be in accordance with HIPAA federal regulations, state regulations and hospital policy. The Health Information Representative interacts with patients in person or by telephone and obtains appropriate authorization for release. The Health Information Representative converts paper medical records to scanned documents to ensure readability, correct indexing, proper orientation and accessibility.
JOB RESPONSIBLITIES

  • Verifies that all requests meet the parameters for release in accordance with HIPAA federal regulations, state regulations, and hospital policy to ensure patient confidentiality. This entails knowledge of when a written authorization is required and when information can be released without patient authorization. Obtains authorizations as necessary.
  • Performs release of information for continuity of patient care in urgent situations in an expeditious manner.
  • Analyze medical reports and information against appropriate quality standards according to state, federal and accrediting agencies requirements for accuracy, completeness, clarity, legibility, timeliness, authentication, date, time restrictions, and content. Notify providers, via the Electronic Medical Record (EMR), for any deficiency noted and record all deficiencies in the appropriate application.
  • Responds to all requests for information from various requestors such as third-party payors for reimbursement, quality review, patients, health care providers, disability, insurance companies, attorneys, and subpoenas. Meets all due dates in processing requests.
  • Maintains an accurate computerized log in Hyland of requests received with the status or completion date. Scans requests and authorizations into Hyland.
  • Demonstrates familiarity with the Electronic Medical Record (EMR). Knows where to access information and print all types of information from various EMR systems. Photocopies records in hard copy. Dependent on site, creates imaging discs of radiology film studies for release and uploads outside imaging into PACS for storage and physician review.
  • Educates patients and other requestors on how to request medical records. Delivers medical records throughout the hospital as needed and in accordance with hospital policy and procedure.
  • Disassemble, identify and prepare paper medical records into specified document types.
  • Analyze content of the paper record to determine if information needs to be scanned.
  • Scan medical record using scanner into the appropriate Electronic Medical Record (EMR).
  • Perform quality assurance review on scanned document images.
  • Attach patient name to each scanned record and properly identify document type for correct indexing.
  • Verify medical record has been properly scanned and indexed to patient chart and prepare paper record for destruction.
  • Ability to utilize computer systems- MS Office with emphasis on Excel, OnBase, Sunrise Clinical Manager (SCM) or similar EMR

JOB QUALIFICATIONS
Experience: Minimum of 2-3 years' experience in a medical record department/medical office setting with familiarity in working with diverse facets of medical record activities.
Education: High School Diploma or GED
Special Skills: Strong organizational skills, ability to multi-task and utilize critical thinking skills to overcome obstacles, strong communication skills
General office equipment experience (i.e. copy machine, fax machine)
Training: Minimum of 2-3 years' experience in a medical record department/medical office setting with familiarity in working with diverse facets of medical record activities.
Pay Range
$17.00 - $24.15
Placement within the identified pay range is based on individual and market factors including, but not limited to, experience, education, credentials (including licenses and certifications), geographic location, market competition, skill set (including market availability of required skills), assigned/anticipated job tasks, and level of responsibility.
