We are seeking a dedicated and experienced Registered Nurse (RN) Licensed in the state of Florida to join our team. In this role, you will be responsible for managing chronic care patients and providing support through a 24-hour patient care phone line.
Responsibilities:
1. Chronic Care Management:
- Provide comprehensive care management for patients with chronic conditions.
- Conduct assessments, develop care plans, and monitor patient progress.
- Coordinate with healthcare providers and community resources to ensure continuity of care.
- Educate patients and caregivers on managing chronic conditions, medication adherence, and lifestyle modifications.
2. After-Hours Support:
- Answer calls from patients and caregivers outside of regular business hours.
- Assess patient concerns and provide appropriate guidance, triage, and escalation as necessary.
- Document all interactions accurately and thoroughly.
- Follow up with Patients as necessary.
3. Clinical Support
- Work closely with the primary care doctor, other LPNs, medical assistants, and administrative staff to ensure coordinated patient care.
- Coordinate referrals to specialists or additional diagnostic tests as needed. Submit medication refill and preauthorization as necessary.
- Participate in case conferences and team meetings to discuss patient care plans and interventions.
- Communicate effectively with patients, caregivers, and healthcare providers.
4. Compliance and Quality Assurance:
- Adhere to regulatory requirements and standards of practice.
- Ensure compliance with documentation, coding, and billing guidelines.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements:
- Active Registered Nurse (RN) license in the state of Florida.
- Minimum of 2 years of clinical experience, preferably in chronic care management or a related field.
-Must have a stable internet and phone connection or the ability to commute to our office in Carolina
- Strong assessment, critical thinking, and communication skills.
- Ability to work independently and collaboratively in a remote setting.
- Proficiency in using electronic health records (EHR) and telehealth platforms.
- Commitment to providing high-quality, patient-centered care.
- Availability to answer calls during evenings, weekends, and holidays.
Preferred Qualifications:
-Experience with Chronic Care Management (CCM) or related specialty.
- Experience in telehealth or remote patient monitoring.
- Familiarity with Medicare guidelines and reimbursements.
