This position is open to California Licensed LVNs.
SUMMARY: The Licensed Vocational Nurse (LVN)-Transitional Care provides outreach and support to Open Door Community Health Center (ODCHC) patients that are transitioning from hospital. The LVN facilitates appropriate use of resources, access to services and communication with the health care team. Through this targeted team approach, it is expected that appropriate health care utilization and health cost reductions will occur.
Key Tasks and Responsibilities:
  • Daily review and monitoring of hospital discharges.
  • Conducts follow up calls to patients discharged from hospital or Emergency Department for assigned sites, utilizing lists provided.
  • Oversees and reconciles all acute transitions of care.
  • Accesses clinical records in hospital EHR to provider for clinical staff review
  • Assist nursing staff by obtaining and attaching relevant clinical information.
  • Serves as point of contact for partners such as: Providence, MRHC, and others.
  • Attending meetings as required by the Supervisor and serving as support in key projects.
  • Support the interdisciplinary team approach to ensure effective resource utilization, as well as quality and cost-effective outcomes.
  • Coordinate internal and external resources for the individual member.
  • Perform other duties as assigned by Supervisor and/or Manager.

QUALIFICATIONS AND EXPECTATIONS:
  • Ability to work as a member of a team to communicate and/or solicit pertinent information to and from other team members and support team decisions.
  • Strong interpersonal skills and the ability to establish constructive professional working relationships among a diverse workforce.
  • Ability to work independently with little supervision.
  • Ability to communicate effectively in multiple ways (written/email/etc) and exercise sound and responsible judgment.
  • Demonstrates age-appropriate communication and interview techniques.
  • Excellent interpersonal skills, written and verbal.
  • Demonstrates flexibility and adaptability.
  • Strong computer skills with specific aptitude in Microsoft Office Suite and/or Electronic Medical Records (EMR).
  • Ability to establish constructive working relationships with all levels of management and employees in a staff of varied and diverse backgrounds.
  • Adheres to the values of quality, dignity, respect, responsibility/accountability, safety, cooperation, honesty, compassion and integrity.
  • Ability to handle difficult or confrontational situations in a calm, consistent, and equitable manner.
  • Ability to read, analyze, and interpret general publications.
  • Ability to effectively represent ODCHC's interests in the community and maintain effective working relationships among co-workers, public, private, and professional groups.
  • Demonstrates an understanding of and functions as LVN in accordance with California Vocational Nurse Practice Act.
  • Skilled in patient assessment and community based resources.
  • Adheres to ODCHC's policies and procedures.

EDUCATION AND EXPERIENCE:
  • High school diploma or GED.
  • Completion of an LVN training program leading to licensing as Licensed Vocational Nurse.
  • Previous clinical experience preferred.
  • Previous experience or training in motivational interviewing preferred.
  • Minimum of one year of experience at ODCHC.
  • Two years of experience in a similar role; or an equivalent combination of education and experience.
  • Bilingual in Spanish preferred.

CERTIFICATES, LICENSES, REGISTRATIONS:
  • Current LVN license for the State of California.
  • Current Basic Life Support (BLS)/CPR Certification.

SUPERVISORY RESPONSIBILITIES: None.
SUPERVISION AND SUPPORT: The LVN - Transitional Care reports directly to the Registered Nurse - Triage Manager and is an integral member of the Clinical Services team.
PHYSICAL REQUIREMENTS: This is largely an office-based position and the physical requirements described are representative of those needed to successfully perform the essential duties of the position. Reasonable accommodation will be made to allow otherwise qualified candidates to perform these functions.
  • Ability to communicate via telephone, video, and/or in-person.
  • Ability to read documents, computer screens, forms.
  • Ability to remain stationary for extended periods.
  • Ability to lift, carry, or otherwise move up to 25 pounds.
  • Ability to use keyboard and view computer screens for extended periods.
  • Ability to travel locally and long-distance as needed.
  • Ability to move around offices and clinics as needed.
