Then consider our Rehabilitation Wellness AideFull Time opportunity. Tx:Team invites you to join our team in Mount Pleasant, SC at the Senior Living community, Somerby Mount Pleasant. This position includes a great mix of leading senior fitness classes and supporting the team with various support and administrative tasks in the department.
A rehabilitation provider since 1983, Tx:Team brings specialized Geriatric Physical, Occupational, and Speech Therapy Services to the Somerby Mount Pleasant community.
The details:

• The beautiful campus includes three living options for seniors: Independent Living, Assisted Living, and Memory Care. At Somerby Mount Pleasant, the mission is to inspire and nurture successful aging and our multi-disciplinary rehabilitation team will play an important role in keeping residents as independent and healthy as possible.
• Somerby Mount Pleasant rehabilitation department includes a beautiful gym space, indoor pool, and access to community rooms, providing the optimal atmosphere for our treatment model for therapy services which includes 1:1 patient care and a therapy-led onsite fitness program.
• Patient care schedule is Monday - Friday with no evenings, weekends, or holidays.
• Opportunity to motivate residents with the goal of keeping them active. Get the residents moving with a variety of classes within the Tx:Team Fitness Program.

The ideal candidate:

• One (1) year experience in a rehab/physical therapy clinic environment
• Prior experience with fitness or wellness classes with the geriatric population or certification in fitness

The requirements:

• High school diploma or GED
• Experience with leading fitness and wellness classes
• Prior therapy clinic or healthcare related experience a plus

The responsibilities:

• Provides patient related support to therapists and team
• Leads fitness and wellness classes, i.e. strength, core, balance
• Schedules patients based on plan of care designated by therapists.
• Responsible for intake information and forms for patients
• Scans scripts and intake information and ensures receipt through Forcura app
• Completes paperwork and updates patient files
• Answers phone and communicates with physician offices, other providers, patients, community personnel, and resident family members.
• Transports patients to and from therapy department if needed
• Maintains and prepares department supplies with pre/post patient room equipment set-up related to daily operations
• Orders and monitor equipment according to established procedures
• Maintains and clean all therapy/exercise equipment
