JOB SUMMARY
Practices the profession of Occupational/Physical/Speech Therapy including evaluating, establishing goals, developing therapy plans and providing treatment to patients with disabilities or limitations. Responsible also for patient and family education, timely and accurate documentation and ongoing communication with physicians and other healthcare professionals as necessary.
RESPONSIBILITIES
Essential Functions
1. Demonstrates commitment to the mission of Swedish Covenant Hospital and demonstrates a service orientation and adheres to all responsibilities and standards of the Hospital.
2. Responsible for evaluating, establishing and updating appropriate goals and treatment plans and providing therapy to inpatient and outpatients with physical disabilities.
3. Appropriately treats patients per program as designed using therapeutic techniques and modalities.
4. Maintains timely and complete documentation in accordance with department policy. Keeps timely and accurate records of patient charges.
5. Interfaces with members of the medical staff and other members of the healthcare team. Is responsible for assuring good communication, problem solving and follow-up to identified needs and concerns of other team members.
6. PT & OT supervise PTA/COTA, Students and Volunteers. Supervises students and volunteers as requested.
7. Continues own professional education through attendance at professional meetings, conventions and courses outside the hospital as well as reviewing current literature.
8. Helps to maintain treatment area in safe, clean and orderly condition. Attends to status of equipment and supplies in clinic, reporting need for ordering as well as maintenance.
9. Ensures that all new orders are initiated and treatment provided (according to department standards) for rehab patients (applies to registry therapists working on weekend).
10. Maintains department productivity standard.
11. Must work at least 2-3 times a quarter.
12. Follow through with hospital/department protocols/requirements such as, but not limited to RAVE, completing competencies and attending mandatory meetings.
13. Bring patient issues/departments issues to the attention of your immediate supervisor.
14. Assures that all patient issues are resolved prior to end of shift or "handed off" to appropriate person.
15. Participates in community re-entry outings with patients outside of hospital grounds for therapeutic purposes.
16. Participates in all aspects of department operation as requested by Rehab Management.
PATIENT CARE/AGE SPECIFIC RESPONSBILITIES AND QUALIFICATIONS
Recognizes needs and behaviors of a variety of disability, age and ethnic groups and is able to tailor programs appropriately. Understands the normal aging process.
ACCESS TO PROTECTED HEALTH INFORMATION
Computer system access is assigned based on managers access level request and on the minimum necessary as described in this job description. Approval for level of access is given by the system administrator or manager of the department that has ownership of system/module. See Computer system Access controls policy.

QUALIFICATIONS/BASIC JOB REQUIREMENTS
• Graduate of AOTA approved school of Occupational Therapy/Graduate of APT approved school of Physical Therapy, approved school of Speech/Language Pathology
• No experience required
• Good communication skills: an understanding and ability read and write English
• Good team player, sound judgement, patient-focused work ethic
• Good organizational and time-management skills
• Ability to work independently

• Current Illinois license or license eligible

ENVIRONMENT
• Sitting: 15%, Standing/Walking: 85%
• Frequently involved in transferring patients. Minimal other regular lifting, carrying or moving objects.
• Repetitive tasks include work-processing and typing approximately 0-5% of time
• Exposed to blood/body fluid and airborne disease during direct patient care
