GENERAL DESCRIPTION OF POSITION
Rehabilitation Coaches must be Certified as Qualified Behavioral Health Providers and deliver clinically appropriate interventions as ordered on the Individual Plan of Care. The Rehabilitation Coach works within a Treatment Team and under the direct supervision of a Mental Health Professional to provide therapeutic interventions with a Recovery-oriented, person-centered approach to assist members in re-building their lives.
ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Rehabilitation Coaches work as para professionals under the direction and supervision of a Mental Health Professional.
2. Understand member treatment plans and prescribed interventions.
3. Effectively deliver and document interventions as prescribed.
4. Submit all documentation by the end of every shift.
5. Communicate daily with MHP's with any concerns/progress of members.
6. Serve as a lead QBHP within the CSSP Program
7. Serve as a liaison between members and community and other resources.
8. Assists members who wish to prepare for and secure jobs.
9. Transport members as directed by supervisor (s).
10. Assist members in maintaining a safe, clean, healthy treatment/residential environment.
11. Collaborate with other members of treatment team/other shifts assuring communication of vital member and other information.
12. Serve as an advocate, resource and role model for members.
13. Maintain healthy relationships with members, always respecting appropriate boundaries.
14. Maintain current certifications as a QBHP to include Face to Face MHP Supervision every 14 days with a minimum of 12 contacts every 90 days.
15. Must complete mandated Inservice training once per 12 month period. Must have a total of eight hours every twelve months beginning with the date of certification as a QBHP
16. Must complete co-occurring and CSSP training.
17. Complete all other annual training as may be mandated by Birch Tree.
18. Adhere to Birch Tree requirements for attendance and punctuality.
19. Organizational, communication and time management skills.

20. Holder of current driver's license and possess the ability to transport members in company vehicles on an as needed basis.
21. Must follow all CSSP Guidelines regarding transport Logs
22. Self-motivated, able to work harmoniously in a team-oriented, culturally diverse environment.
23. The ability to work in a constant state of alertness and in a safe manner.
24. Must be able to work shifts assigned based on the needs of the members and/or branch. Every effort will be made to maintain a consistent schedule but there may times that schedules change on short notice.
25. Must be able to do housekeeping duties, such as, but not limited to, laundry, sweeping, mopping, cooking meals, kitchen clean up, bathroom cleaning, etc.
Perform any other related duties as required or assigned.

EDUCATION AND EXPERIENCE
High school diploma or equivalency.
REQUIRED CERTIFICATES, LICENSES, REGISTRATIONS
Must have a High School diploma or GED and a valid driver's license. Must be Certified as a QBHP and must have completed CSSP Training
