Job description

The Per Diem Rehabilitation Coordinator provides coverage for the Van Munching Admissions coordinator, PPS coordinator and Case manager/Social worker.

The PD Rehab Coordinator is responsible for:

  • the identification of, and overall clinical and financial screening of all applicants referred for admission to the acute rehabilitation beds on the Van Munching Rehabilitation Unit at The Stamford Hospital when covering for admissions. 
  • managing the discharge process from VMU when covering for CM/SW
  • submission of documentation and billing to receive appropriate Medicare payment according to CMS guidelines when covering PPS

 

MAJOR ACCOUNTABILITIES/CRITICAL RESPONSIBILITIES:

  1. On-site clinical liaison to identify and assess all patients referred for acute rehabilitation.
  2. Act as the source to assess patients who meet program criteria and will benefit from admission to the VM Rehab unit through:
  • Early identification of candidates/clients
  • Education of patients, families, physicians, and facilities both internal and external of admission criteria
  1. Liaison between case management and social work to coordinate a timely discharge from acute care to Van Munching,
  2. Consultant for social work, case management and hospital personnel re: acute rehabilitation unit criteria, etc.,
  3. Assist in the discharge planning process for Van Munching Rehabilitation Unit
  4. Serves in a consultant capacity to referring facilities with attention to a candidate's

rehab potential.

  1. Provide facility tours and other community marketing and outreach activities
  2. Recommends alternative referrals to those who are denied admission
  3. Completes all pre-admission screening and documentation to assure regulatory compliance
  4. Continuing education for nurses, physicians, therapists, case managers and social workers regarding the appropriate criteria for admission to the rehabilitation unit,
  5. Patient and family teaching regarding the rehabilitation process and the goals of treatment,
  6. Data entry for UDS
    1. Coordinates the process of providing accurate and timely data to the appropriate entities involved in the Prospective Payment System for Medicare patients.
    2. Responsible for clinical data collection, entry, transmission, and analysis to ensure compliance with the requirements of the Medicare Prospective Payment System
    3. Ensures the presence of supporting documentation and provides staff education to ensure compliance with CMS guidelines.
    4. Coordinates all patient information to be submitted to CMS for Medicare reimbursement.
    5. Responsible for Utilization Review and involved in team conferences to maximize LOS efficiency. 
    6. Coordinate and monitor compliance with 75% rule
    7. Responsible for monitoring patient charts for correct coding, electronically transmitting data in a timely manner and communicating with a multidisciplinary team to create and carry out patient treatment plans. Monitor program compliance with changing for Medicare Services (CMS).

 13.Maintains, analyzes, and educates on quality metrics associated with the acute rehabilitation program

14.Re-education of nurses, physicians, therapists, case managers and social workers regarding the new admission criteria under the Prospective Payment System

15.Attendance at interdisciplinary rounds on hospital units to screen and discuss potential rehabilitation patients

16.Facilitates the planning, coordination discharge process for a specified patient population in the inpatient rehabilitation facility with the multidisciplinary team

17.Provides ongoing support and expertise through comprehensive assessment, care planning, implementation, and overall evaluation of individual patient needs.

18. Conducts patient and family education regarding the rehabilitation process and the goals of treatment, discharge plan, and financial coverage.

19.Conducts education to the interdisciplinary team regarding post-acute settings, discharge process, and insurance regulations.

20.Determines and ensures that the services recommended are appropriate, reasonable, and necessary

21.Facilitates care coordination to ensure a smooth transition to referring facilities and home care management

22.Works effectively with physicians, nurses and both internal and external case managers to develop and provide a cost-effective plan of care

23.Completes the insurance authorization process by providing an initial plan of care and securing insurance authorization for services rendered and coordinating services following discharge from the acute rehab unit.

24.Facilities patient care and family meetings to discuss plan of care, discharge needs and IDT recommendations for appropriate disposition setting

25.Performs other related duties as assigned or requested
