Registered Nurse(Rn)/Licensed Vocational Nurse(Lvn/Lpn)

Home health RNs/LVNs:

Who have experience in home health nursing to join our team. We're looking for an individual who has previous experience working in a home health environment, and is able to work with individuals and families in their homes. This is an exciting time to join our team, as we are in the process of expanding our services and allowing nurses more opportunities to succeed and advance their careers.

Home Health work experience preferred.

Responsibilities:

Comfort and support the patient and family.

Prepare the patient for opening cases, discharge and set up needed equipment in the home setting.

Evaluate the needs of the patient and his or her family and help to coordinate care with other professionals.

Maintain documentation according to state regulations.

Provide care as directed by a physician.
