JOB SUMMARY:
The Registered Nurse provides general counseling, education, and guidance services to our triage customers within the Nurse Advice Line.
ESSENTIAL DUTIES
1. Performs an individualized assessment; designs a care plan ensuring goal achievement.
2. Completes the patient medical file, including health-related aspects.
3. Gathers laboratory results, medications, and primary care physician information.
4. Provides health-related education and coaching, in accordance with the established clinical guidelines, laws, and regulations.
5. Promotes conduct modifications in the patient through coaching, utilizing motivational interview techniques.
6. Refers to patients according to specialized needs. For example, mental health, nutritional evaluation, medical therapy management, smoking cessation, and others.
7. Registers new patient data and interventions in the provided software.
8. Maintains effective communication amongst the different Program members, medical plans, and practitioners.
9. Participates in educational meetings and case discussions.
10. Ensure confidentiality and compliance with all state and federal regulations applicable to Health Call Centers; including HIPAA and internal policy for disposing of confidential information material.
11. Notify any exposure of Protected Health Information to the supervisor immediately.
12. Comply with medullar & functional training and all procedures established by regulatory agencies, contractual requirements, and any applicable certification standards (including but not limited to URAC, amongst others).
