Job Summary:
We are seeking a dedicated and compassionate Travel Registered Nurse Case Manager to join our healthcare team. As a Registered Nurse Case Manager, you will be responsible for providing comprehensive and coordinated care to patients, ensuring their needs are met throughout their healthcare journey. Responsibilities:
- Conduct thorough assessments of patients' medical history, current condition, and healthcare goals
- Develop and implement individualized care plans based on patients' needs and goals
- Coordinate with interdisciplinary healthcare team members to ensure seamless care transitions
- Monitor patients' progress and adjust care plans as necessary
- Educate patients and their families on disease management, treatment options, and preventive measures
- Collaborate with physicians and other healthcare professionals to provide optimal patient care
- Document patient information accurately and maintain confidentiality in accordance with HIPAA regulations
- Provide emotional support to patients and their families throughout the healthcare process
- Facilitate discharge planning and coordinate post-discharge care services
Qualifications:
- Valid Registered Nurse (RN) license in the state of [state]
- Bachelor's degree in Nursing (BSN) preferred
- Strong knowledge of physiology, diagnostic evaluation, and treatment options across various medical specialties
- Familiarity with primary care, acute care, and pediatric settings
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment while managing multiple priorities
- Proficient in electronic medical record systems and other relevant software applications
