Job Description
- Degree: Associate or Baccalaureate Degree of Nursing.
- Education: Graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN), or The Commission on Collegiate Nursing Education (CCNE).
- Experience: Two years of experience after graduation in ambulatory nursing. One year of nurse educator experience in Women's Health within the last three years.
- Licensure: Current, full, active, and unrestricted license to practice as a Registered Nurse.
- Experience with Microsoft Office Suite.
- The HCW shall provide services within the scope of their competencies, the applicable duties provided in the basic contract, and the following: Services shall be consistent with the scope of practice for the profession, including, but not limited to, patient education; professional care in a knowledgeable, skillful, consistent, and continuous manner; documentation of patient care and observations; and emotional support. The HCW shall develop and maintain a comprehensive health education program, in conjunction with performing ambulatory nursing duties.
- Duties as a RN require a full range of knowledge and skill in accordance with assignments under this task order. Services shall be provided in a consistent and continuous manner and shall include such duties as: triage; patient assessment and monitoring; use of patient monitoring and treatment equipment; appropriate nursing care, procedures, and treatments; execution of physicians’ orders within the guidelines of standard nursing practice; documentation of patient care and observations; and patient education and emotional support.
- Make independent nursing practice decisions to plan, organize, develop, and implement methods of providing quality nursing care/nursing service in an ambulatory care setting.
- Provide individual, group, and family/significant other counseling and health teaching in relation to the patient's condition and his/her ability for self-care. Provide for accessibility to various literature resources dealing with subjects related to specific patient health problems. Provide physical and psychological support to patients and significant others, explain procedures and treatments, and promote cooperation among staff, patients, and significant others.
- Maintain liaison with allied health care professionals and community referral services. Coordinate patient health care needs with appropriate referrals as indicated.
- Maintain communications with professional organizations and be familiar with nursing standards relevant to ambulatory care. Implement nursing care in the ambulatory setting using these standards.
- Liaise with appropriate inpatient units as an advocate for continuity of care planning, follow-up, and outcome measurement.
- Possess age specific knowledge and competency appropriate to setting population.
- Promote health maintenance and preventive health initiatives in caring for patients in the ambulatory setting.
- Assist in the planning, organizing, and support of clinical operations to include providing technical support and guidance to paraprofessional nursing staff. Possess knowledge of basic management theory as applicable to the ambulatory care setting.
- Assist in the review and revision of clinic standing operating procedures.
- Perform assessment/data collection in an ongoing and systematic manner, focusing on physiological, psychological, and cognitive status.
- Ensure areas are stocked and properly equipped.
- Assist in planning and instructing licensed practical/vocational nurses (LPNs/LVNs), nursing assistants and ancillary personnel.
- Actively participate as the subject matter expert in the Put Prevention into Practice (PPIP) Initiative and Organizational improvement plan. Develop and implement wellness visit protocol consistent with command PPIP.
- Help maintain good interdepartmental relations through positive communications and work coordination.
- Provide verbal updates to clinic/office managers as to progress and /or issues/problems.
- Provide input into planning and purchasing resources needed to build and maintain a current local health resource center and directory.
- Maintain current knowledge of literature concerning treatment, prevention and other information related to preventive health.
- Plan and implement professional training to clinic staff members, medical providers, and other groups on a quarterly basis and as requested.
- Assist the Health Promotion and Public Affairs Officer to develop and implement a marketing and awareness plan for health issues.
- Assist with developing a partnership with and participate in community wide health initiatives, which are consistent with DOD goals and impact beneficiaries of NHC Lemoore.
- Provide template management to the providers. Educate clinic staff and beneficiaries on the elements of the overall health and wellness direction of the clinic, including training hospital staff members, health promotion and education training, development of educational and training materials.
- Assist in the development, implementation, evaluation and documentation of a comprehensive, population-based wellness program for the enrolled population. Develop presentations and course work for classroom lectures. Lesson plans will be in electronic format. Course work will include a strong emphasis on Management Information Systems skills and learning due to the highly automated nature of the facility. Training shall be provided to members of the clinical and administrative staff on subject germane to wellness and health promotions.
- Review and analyze wellness programs provided to the enrolled population, identify opportunities to improve the programs and recommend corrective action when problems exist.
- Collaborate with the Population Health Program Manager, review and make recommendations as necessary to improve Hedis data.
- Input data to the DD2766 automated flow sheet and place the form in the health records of enrollees.
