About the Role
- The service is community based and involves working directly with elderly clients to maintain their independence and avoid unnecessary hospitalisation.
- TACP Moree assist clients with activities of daily living, wound management, assistance with exercise programs and education and support for clients to be able to stay in the home. Additional support may include taking clients shopping; transport to and from Doctors or health related appointments.
- The RN will coordinate care of clients with assistance of an AIN and EN as part of the team.
- Input and assistance is also available from Physiotherapy and Occupational Therapy services.
- In your role, you will be supported through staff and online education.

Where you'll be working
- The Transitional Aged Care Program (TACP) Moree is part of the Moree Community Health Team. It is supported by nursing and allied health staff.
- Moree is a town of relaxation and rejuvenation, and is situated in the beautiful Gwydir Valley in northwest New South Wales.
- One of the most popular recreational activities to do among locals and visitors is to visit the Moree Artesian Aquatic Centre to enjoy the thermal springs, naturally heated to 41 degrees Celsius. For the thrill seekers, Moree Water Park is a popular attraction in the warmer months.
- Direct flights operate between Moree and Sydney 6 days a week.

Information for Applicants:
All NSW Health workers are required to have completed a primary course of a COVID-19 vaccine which has been approved or recognised by the Therapeutics Goods Administration (TGA).
