**** THIS POSITION IS ELIGIBLE FOR A $10,000 SIGN-ON BONUS ***
The Registered Nurse, Team Lead, Referral Center (TLCSC) is responsible for the oversight and day to day management of the Customer Service Center. Ensures all referrals are appropriate to the requested service line. Communicates with Hospital Home Care Coordinators, internal and external referral sources to assure all required information is obtained and confirmed for referral processing. You must have a New York State Registered Nurse License for this position.
Performance Responsibilities and Standards
1. Promote continuity of care for referred clients (including but not limited to):
  • Evaluate and process referrals from hospitals, physician offices, skilled nursing facilities, community agencies, patients and their families as needed to assist the CSC RN.
  • Answer inquiry calls from clients, families, physician offices, community agencies and insurance companies.
  • Update referral information as appropriate
  • Takes verbal orders from referrals sources

2. Promote quality assurance as it relates to home care planning (including but not limited to):
  • Participate in quality improvement activities related to home care planning and coordination.
  • Adhere to quality standards including the confidentiality of client information.
  • Runs and utilizes capacity information to manage the number of referrals accepted each day.
  • Maintains an organized report of all referrals coming into the agency from hospitals, physician offices and nursing home facilities.
  • Oversee the review of clinical information, orders and other communications to assure referral information is complete and provides high quality information to field staff

3. Other expectations (including but not limited to):
  • Ensure efficient workflow in the Customer Service Center
  • Promote positive relationships among referral sources, patients and families
  • Identify problems and report to Referral Center Supervisor and or Director of Clinical Analytics & Business Development
  • Provide excellent customer service
  • Attending meetings and conferences as required
  • Promote a positive work environment within the CSC department and promote positive communication with the other Team Leaders
  • Performs all other duties as assigned by direct supervisor and/or department director

Requirements
  • Current New York State registered professional nurse license
  • Bachelor's degree from an accredited school of nursing preferred
  • Minimum of two year's acute care experience; one-year home health experience
  • Ability to use critical thinking and to make decisions with confidence
  • Excellent organizational skills with the ability to work independently
  • Excellent communications skills and problem-solving ability
  • Compliance with agency policies and procedures
  • Ability to use a variety of computer software

Physical Requirements
  • Ability to sit or stand at a desk 90% of the day
  • Visual/hearing ability sufficient to comprehend written or verbal communication
  • Ability to express self verbally and in writing
  • Ability to use telephone and communicate over the phone
  • Frequently uses a laptop computer to enter orders and obtain product information
  • Bloodborne Pathogens Exposure Category III - Employee who rarely has contact with blood and body fluids
