Job description:
This is a part-time position with flexible hours. Our ideal candidate
would be working 4- 8 hours a week on an as needed basis (for client
intakes, medication management, training staff on medication admin,
and emergencies by phone). Hours will be negotiable between the needs
of the clients and the schedule of the potential hire.
Upon hire you will be required to submit a background study, upon
clearance you would be able to start. More details will be discussed
during the interviewing process.
Details of assessment:
COMPREHENSIVE CLIENT ASSESSMENT
POLICY:
A thorough, comprehensive and accurate assessment, consistent with the
client’s immediate needs will be completed by a Registered Nurse. The
initial assessment will be completed within 5 days after the
initiation of services.
Client monitoring and reassessment must be conducted in the client’s
home no more than 14 days after initiation of services. Ongoing client
monitoring and reassessment must be conducted as needed based on
changes in the needs of the client and cannot exceed 90 days from the
last date of the assessment
The Registered Nurse will update the assessment and service plan as
needed when changes occur. This would include return home following
hospitalization or stay in nursing facility or changes in condition
that alter the plan of care
PURPOSE:
To determine the appropriate care, treatment and services to meet
client initial needs and his/her changing needs.
To collect data about the client’s health history, (physical,
functional and psychological) and their needs as appropriate to the
home care setting. To make care, treatment, or service decisions based
on information developed about each client’s needs and the
individual’s response to care.
To identify clients medical, nursing, rehabilitative, social and
discharge planning needs.
SPECIAL INSTRUCTIONS:
The Comprehensive Assessment will be completed by a Registered Nurse,
except in situations where therapy services are the only service
ordered by the physician.
The Comprehensive Assessment will include a review of all medications
the client is using. This assessment will identify potential adverse
effects and drug reactions, including ineffective therapy, significant
side effects, significant drug interactions, duplicate drug therapy
and non-compliance with therapy.
In addition to general health status/system assessment, the agency
comprehensive assessment tool will include:
Demographics and client history.
Living arrangement, and support systems
Integumentary status.
Neuro/emotional/behavioral status.
Functional limitations in completing activities of daily living
Assessment of all medications the client is taking and will identify
potential adverse effects, drug reactions, ineffective therapy,
significant side effects or drug interaction, and duplicate drug
therapy. The clinician will also assess for risk of drug diversion.
Vulnerability and Safety Assessment to determine risk of being abused
and risk of abusing others. Plan is developed in compliance with the
Vulnerable Adult Law
Nutritional assessment to identify those at risk due to inadequate
fluid and food availability or willingness of client to follow plan
Equipment needs and ability to manage the equipment
Identification of advance directives, POLST, or specific emergency
response instructions the client has and coordinating inclusion of
wishes into the home care plan of care.
Emergent care data.
Items collected at in client facility admission and discharge.
Functional status is assessed and documented using data elements with
agency specific assessment criteria.
Assessment and documentation are made regarding whether the home
environment is suitable for providing home care.
Discharge planning is initiated, goals are identified, and/or
continuing care needs are recognized.
All clients will have the Comprehensive Client Assessment completed
within five (5) days of the initial visit.
Assessments will be completed by Registered Nurses or other
appropriate licensed health professionals.
Reassessment/monitoring visits are conducted no later than 14 days
after the start of services
Ongoing assessments will be completed based on client needs, change in
condition or client request. Reassessments must be done no more than
90 days after the previous assessment
The monitoring and reassessments may be conducted at the client’s
residence utilizing telecommunication methods based on practice
standards that meet the needs of the client.
The initial and ongoing assessments are the foundation for
establishing a client service plan that addresses client needs and
preferences and provides direction to agency staff
Ongoing monitoring of compliance with the applicable regulations and
policies will be included in the quality management program.'
