Home Town is a growing Home Health and Hospice agency that creates a positive atmosphere for our staff. Home health allows clinicians to slow down, care for patients one-on-one, treat, heal, and educate patients and families in the safety of their own home.

Duties include:

  • Directs nursing and home healthcare as prescribed by the attending physician
  • Reviews all plans of treatments for accuracy, appropriateness and completeness
  • Maintains accurate records of all services rendered to patients
  • Evaluates and regularly reevaluates the therapy and nursing needs of each patient
  • Performs initial admission assessment and evaluation of the Agency’s patients
  • Ability to assess adequately and reassess pain
  • Utilizes appropriate pain management techniques
  • Educates the patient and family regarding pain management
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/healthcare errors
  • Reports to the physician the patient’s response to treatments, including drugs, and changes in the patient’s physical or emotional condition
  • Demonstrates knowledge of medications and their correct administration based on the age of the patient and the patient’s clinical position
  • Follows the five (5) medication rights and reduces the potential for medication errors
  • Teaches, supervises, and counsels the patient and family members regarding the nursing care needs
  • Performs patient care responsibilities considering needs specific to the standard of care for the patient’s age
  • Addresses and supports cultural practices, as long as such practices do not harm others or interfere with the planned course of medical therapy
  • Supports and carries out the Agency’s policies
  • Participates in the Agency’s information management and performance improvement programs
  • Assures that monthly case conferences are done on each patient, recorded, and filed in the patient’s chart
  • Completes annual educational requirements
  • Maintains regulatory requirements
  • Adheres to dress code, appearance is neat and clean
  • Represents the organization in a positive and professional manner in the community
