We're seeking an RN to provide home and facility-based wellness visits. The opportunity allows for autonomy and the ability to work a caseload at your own pace. This position is open for immediate direct hire.
MONDAY THROUGH FRIDAY - DAYLIGHT HOURS

Job Summary:
Provides direct and indirect primary nursing care to assigned participants in both clinic and home-based environments.
Works under the direct and indirect supervision of the Program Director or physician extender but is independent in the application of advanced nursing knowledge and skills.
Manages complex clinical situations for which he/she is responsible.
Supervises both licensed and non-licensed personnel.
Performs assigned duties and responsibilities in accordance with current existing federal and state regulations and established company policies and procedures.
Understands and demonstrates commitment to the company's mission through daily communication with participants, families, community and staff.
Builds a trusting relationship with each participant/family/caregiver, by being available, listening, communicating, and educating via every available forum, such as day center, home visits, phone calls, scheduled and unscheduled clinic appointments, etc.
Requirements:
Must be a Registered Nurse in the State of Pennsylvania
Must have a valid driver’s license and a reliable vehicle
