Position Description:
The Home HealthcareRegistered Nurse (HH RN) utilizes the nursing process to coordinate the interdisciplinary approach to patient care for her/his team of patients. He/she promotes continuity through communication of the patient's plan, condition and progress toward outcomes. The HH RN participates in and demonstrates effective education and support for patients and their families to meet their physiological and psychological needs. He/she practices within the boundaries of the Kansas Nurse Practice Act, specialty standards, and organizational/departmental policies and procedures.
Position Requirements:
Education:
Graduation from an accredited school of nursing required
2 year/Associate degree required
4 year/Bachelor degree preferred
Experience:
3-5 years experience preferred
Home Health experience preferred
Knowledge, Skills:
Analytical, Organizational, Verbal Communication and Computer Systems (computerized clinical documentation) skills required
License, Registration and/or Certification:
Active Registered Nurse (RN) licensure in accordance with the Kansas Nurse Licensure Compact - Required upon hire
Healthcare Provider CPR – (American Heart Association or American Red Cross ONLY) - Required within 7 days from date of hire
