Responsibilities
POSITION SUMMARY: The Post Acute Transitions Coach provides assessment and proactive education and care coordination to patients with one or more chronic conditions through both face-to-face and telephonic contact. The Case Manager utilizes clinical skills in a collaborative process to assess, educate, plan, implement, coordinate, monitor and evaluate options and services to facilitate appropriate healthcare outcomes for patients participating in the Transitions program. The coach works with the health care team and patient to develop a plan of care to promote patient self-management and reduce readmissions to the hospital using adult learning principles and self efficacy theory.
