Homecare Nurse - Field - Case Manager RN

Looking for an experienced Homecare RN's for Mt Laurel/Camden county, NJ

Performs Patient assessments

Take vital signs

Wound care experience

Foley catheter experience

Accurately assesses patient's appropriateness for home care, learning needs and progress toward goals

Develops the plan of care for the patient

Accurately completes the OASIS assessment

Ensures appropriate utilization of services

Maintains visit productivity standards

Demonstrates knowledge of insurance reimbursement

Completes discharge assessments

Performs laptop transfer of data twice daily

Transfers OASIS assessment within 24 hours

Completes re-visit documentation and transfers back

Uses age appropriate teaching methods for patients and families

Evaluates teaching plan and revises as necessary

Documents patient education tools used

Assumes responsibility for Case Management.

Develops/implements/evaluates plan of care based on evidence-based knowledge

Revises care plan as indicated

Prioritizes care needs

Demonstrates knowledge of home care regulations and insurance reimbursement guideline

Shows evidence of interdisciplinary communication

Performs HHA supervisions per standards

Ensures appropriate utilization of services

Follows scheduling process

Develops/implements plan of care based on evidence-based knowledge

Education/Experience:

Valid NJ State RN License OR Compact License

Graduate of an accredited nursing program.; BSN or progress towards BSN preferred

Minimum of 1-2+ years of home care experience in a Medicare certified agency OR Med Surg setting

BLS required
