Minimum of one year experience in health care setting, preferably home health, hospital, or long-term care Adhering to the Best Practices established by the Board of Nursing and CAHC Accreditation Standards Open communication with other Team Members, Clients, and Family Provide input regarding placing care partners appropriately with clients Supervision of CHHA team members for on-campus / off-campus clients Making monthly follow-up calls/visits to ensure adherence of Care Plan Evaluate effectiveness of care provided by CHHAs with timely completion of 60-day skills assessments during on-site visit Ability to join on-call rotation Our team members are the most important asset of FellowshipLIFE.
