Now Hiring Registered Nurses in Home Health
We’re Looking for Registered Nurses Who:
- Will provide the best patient care around
- Love working independently, but want to have a team who is always there to support them when needed.
- Are great communicators, both written and orally.
- Want to work part-time or fulltime in home health in the Green Bay, and surrounding areas.

Requirements:
- Currently licensed in Wisconsin as a Registered Nurse.
- Current American Heart Association CPR Certification.
Job Duties include:
- Acts as Case Manager and assumes responsibility to coordinate patient care
- Complete and document a thorough assessment of patient and develop individualized patient treatment plans
- Work with clinical staff, patient and families to maximize patient outcomes
- Document the course of treatment in a clear, concise manner, including a discharge summary utilizing the prescribed format.
- Instruct Licensed Practical Nurses in treatment procedures to be utilized and provide supervision
- Collaborate with clinical team to develop and educate patient and/or care givers on safe discharge plan
