This is an important role that requires accuracy, attention to detail, and a passion for providing exceptional healthcare. Assessment of Hemodialysis patients; leading to development, implementation and evaluation of plan of care. Administers, monitors and documents therapeutic interventions and regimens. Effectively manages rapidly changing patient stations, including delegating care to other department members. Assist providers with patient-related needs before, during, and after hemodialysis care; making sure their needs have been met in a timely, efficient, and caring manor. Triage and set up extra care at other facilities for the patients. Obtain lab specimens, perform lab competencies and quality controls on a regular basis. Educates patients and/or family members regarding disease process, individual care needs, safety issues, etc. Attend daily huddles, monthly meetings and participates in daily clinical decision makingPerform other duties as requested by Supervisor, Director and/or Providers.
