Position: RN Case Manager (Home Health Agency)
Location: 440 E Sample Rd., Pompano Beach, FL 33064 (Hybrid Remote)
Job Type: Full-Time
Key Responsibilities of the RN Case Manager:
Provide guidance to nursing staff.
Develop individualized care plans.
Coordinate with healthcare professionals to ensure quality care.
Monitor patient progress and adjust care plans as needed.
Educate patients and families on health care needs, conditions, and options.
Ensure compliance with all state and federal regulations.
Qualifications for the RN Case Manager:
Valid RN license.
At least 1 year direct care experience with geriatric population.
Experience in home health care preferred.
Strong communication and organizational skills.
Ability to work independently and as part of a team.
