POSITION HIGHLIGHTS
Our RN-2 Dermatology and Care Management is a unique cross-functional RN2CM role that advances our mission to improve the health of the public by intersecting areas of dermatology and care management for our South Lake Union patient population.
The RN2CM supports the growth of our SLU Dermatology patient population, as well as manages complex patient cases involving chronically ill patients by implementing treatment plans and facilitating patient's access to appropriate health care services and community resources. They educate patients, identify and remedy gaps in patient therapies, and manage therapy for patients enrolled in Care Management, which includes assessments, planning, and evaluating the patient's response to treatment.
DEPARTMENT DESCRIPTION
Located on the corner of Dexter Avenue N. and Republican Street, UW Medicine Primary Care at South Lake Union offers primary care appointments six days a week and a full spectrum of primary care services including family medicine, integrated behavioral health, dermatology, obstetrics and gynecology, and sports medicine. We also house designated specialty care from the UW Diabetes Institute and the Karalis Johnson Retina Centers, as well as an on-site pharmacy, laboratory, and digital X-ray.
