Qualifications:
- Completion of an accredited professional nursing program
- Current Minnesota Registered Nurse License
- BLS certification
- One to three years nursing experience preferred
Job Responsibilities:
Practice of Professional Nursing includes but is not limited to:
- Required to work to the fullest scope of RN Licensure
- Works as a primary point of contact for healthcare providers regarding anticoagulation (Warfarin, Eliquis, Xarelto, Pradaxa, LMWH)
- Maintains high standards of professionalism in communication with clinicians and the staff
- Assumes primary management of medication therapy to include:
- Does adjustments that require strong critical thinking skills and knowledge (assessing patients' medication, diet, health changes, etc.)
- Ordering medication refills and labs
- Initial and ongoing education related to HIGH-RISK medications to patients and families, either in-person or over the phone
- Advice on adverse reactions such as bleeding or clotting
- Strong knowledge base on multiple interactions to include:
- Medications, Class A, B, C, and D interactions, health, diet, etc.
- Provide recommendations in dosing to providers in certain circumstances (last minute procedures, dental procedures, no maintenance plan dosing, etc.)
- Coordinates with providers based off referrals.
- Appropriate medication based on diagnosis, lab values, etc.
- Goal range based on indication for anticoagulation
- Duraction of therapy
- Provide pre and post procedure recommendations
- Assess individual patient's clot risk, bleeding risk and procedure bleeding risk and carefully weigh benefit versus risk
- Provides recommendations to hold warfarin and/or bridge with LMWH
- Teach how to properly administer LMWH and advice on risk of bleeding
- Dosing based on weight and labs
- discuss and advise medication interruption with proceduralist at OMC and also other facilities
- Monitor patient compliance of high risk medication, including timeliness of lab monitoring
- Perform extensive chart review not only in Epic, but legacy chart as well
- Assist in policy and guideline revision and other duties assisted as necessary
- Attend various department meetings as the experts in Anticoagulation
- Provide shadow based training to pharmacy students and requesting providers
- Participate in transfer of care services
- Answer phone calls and advise patients, family members, providers, pharmacist, and outside facilities
- Send home testing referrals and continue ongoing management of warfarin
- Order supplied based on office needs
- Other duties as assigned.
