Key Responsibilities:

• Provide comprehensive care management services in various practice areas.
• Attend weekly office meetings, work remotely, and assist clients at medical appointments and community engagements.
• Engage in considerable writing tasks, requiring clear, non-technical language.
• Collaborate effectively within a team environment to support client needs.

Qualifications:

• Licensed Registered Nurse with a minimum of ten years of postgraduate experience in case management.
• Experience in Psychiatric and Rehabilitation fields preferred.
• BSN or higher qualification is desirable.
• Demonstrated proficiency in using various electronic information applications, EMR systems, and Microsoft Office Suite.
• Strong internet connectivity for optimal performance.
• Valid driver’s license and a car for client-related travel.

Key Attributes:

• High level of flexibility and accountability, particularly for Per Diem roles.
• Excellent written and verbal communication skills.
• Ability to work independently and as part of a team, meeting deadlines effectively.
• No requirement for hands-on care.

Work Schedule:

• Somewhat flexible; predominantly Monday to Friday, 8:30 am – 5:00 pm.
