Position OverviewThe Medicare Annual Wellness Visit Nurse is responsible for implementing Annual Wellness Visits and Chronic Care Management services at their assigned PHN sites. Annual Wellness Visits for Medicare patients consist of Initial Preventive Physical Exam visits (IPPE or “Welcome to Medicare”) and Annual Wellness Visits (AWV) both initial and subsequent. Chronic Care Management (CCM) services consist of monthly activities designed to intensively manage chronic conditions of qualified patients and to coordinate all care that patient receives. Location and Hours of PositionThis position will travel between two of our locations in the Shenango Valley, Buhl Farm Community Health Center and Green Street Primary Care. The general office hours for this position are Monday to Friday 8:30 am - 5.00pm.NO WEEKENDS CompensationPosition starts at $28.00 per hour, however, PHN recognizes experience, education and site location when determining wage.
