Responsibilities:
- Perform patient assessments and develop individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor and record patient vital signs and medical information
- Collaborate with interdisciplinary healthcare team members to provide comprehensive hospice patient care
- Educate patients and their families on hospice care, medications, conditions, and self-care techniques
- Assist with equipment set-up, medication ordering and supply ordering and delivery.
- Provide acute pain management and assist with post-operative care
- Utilize medical terminology to accurately document patient information
Skills:
- Proficient in patient assessment and care planning
- Knowledge of medical terminology for accurate documentation
- Experience in critical care settings, such as intensive care units or emergency departments
- Familiarity with healthcare practices and regulations
- Ability to provide quality care in assisted living facilities or long-term care settings
- Experience in acute pain management techniques
