Location: Greater Eugene/ Springfield Area

Job Description: We are currently seeking a dedicated and compassionate Registered Nurse (RN) to join our team and provide exceptional in-home care to our clients. The ideal candidate will be committed to delivering high-quality healthcare services with a focus on promoting wellness.

Responsibilities:

• Conduct comprehensive assessments of clients' health status and develop individualized care plans.
• Administer medications, treatments, and procedures as prescribed by physicians.
• Monitor and evaluate clients' progress and response to treatment.
• Provide education and support to clients and their families regarding health management and preventive care.
• Collaborate with interdisciplinary team members to ensure continuity of care and optimal outcomes.
• Maintain accurate and timely documentation of client assessments, interventions, and outcomes.

Qualifications:

• Current licensure as a Registered Nurse (RN) in the state of Oregon
• Minimum of 2 years of clinical experience in a healthcare setting, preferably in home care or a related field.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and collaboratively within a multidisciplinary team.
