The Registered Nurse (RN) primarily provides patient care including intake assessment and discharge teaching for patients. The position may also serve as a Case Manager to a group of assigned wound care center patients/providers, depending on the strategic partnership with the assigned hospital. This position is also responsible for providing wound prevention and treatment to the facility’s patients as prescribed by the provider and in accordance with established facility protocol and local, state, and federal regulations. Additionally, the nurse will provide the facility with training, education, and oversight as needed or requested.
Wound care experience is preferred but no mandatory as we will offer on the job training!
Duties and Responsibilities:
- Provides patient care to wound care center patients and handles patient intake and discharge processes as assigned and according to the clinic and company standards.
- May also serve as Case Manager to a group of patients/providers as assigned.
- Performing the initial nursing assessment, communicating findings with the assigned provider, documenting care plans, and identifying patient care situations that require intervention.
- Participating in quality improvement initiatives, providing education to the patient or caregiver, and serving as a patient advocate in the delivery and coordination of patient care.
- Collaborates as needed with other healthcare providers, Home Health Agencies, Long-term Care facilities, family members, and center Medical Director regarding patient needs.
- Performs other duties/responsibilities as assigned.
- Ability to educate patients and families about wound management, both from a clinical and functional standpoint.
- Demonstrates the ability to educate and support clinical staff responsible for the care of patients with wounds.
- Practices professional collaboration with patients, visitors, physicians, and staff.
- Communicates appropriately and clearly to physicians, staff, management, and administrative teams regarding best practices in wound care and program implementation.
- Maintains a good working relationship both within the department and with other departments.
- Ability to perform wound assessment, reassessment, and management for all patients per policy. This includes neonate, pediatric, geriatric, and the general population.
- Provides leadership and direction in accordance with organizational goals and objectives.
- Attends seminars, conferences, continuing education classes in the areas of diabetes/wound prevention, diabetes/ wound management, and ostomies. Participates in professional societies. Stays current with changes such as new advances/treatments in wound and ostomy care.
- A resource person to physicians, nurses, and other hospital staff and provides educational support about diabetes and wound/ostomy management.

Qualifications: Registered Nurse with current license. Wound Certification, and previous leadership position preferred. Maintains competencies in BLS.
Computer Skills: Must be able to use all Microsoft Office functions and have a general understanding/experience with current Electronic Medical Record (EMR) platforms.
Work Environment: The position will not be required to work under any extreme conditions but may be required to work around certain chemicals and will be exposed to smells. The position will work around moderate office noise levels.
Physical Demands: The position requires standing, walking, and sitting. This person must be able to talk and hear. Finally, the position may require light lifting and pulling functions.
