Visits residential programs and offices on a routine basis and as specific needs arise. RN will also be responsible to provide initial new hire medical orientations/trainings as well as ongoing staff training as needed.
Requirements:
Must be at least 18 years of age or older
Must have a High School Diploma or Equivalent
Must pass a Criminal Background Check
Licensed Registered Nurse will not be listed on the Central Registry
Graduate of approved school of Practical Nursing
Licensed and currently registered in the State of New Jersey with a current valid license in good standing as an RN issued by the NJ Board of Nursing
Minimum of 2 years’ experience as a Registered Nurse

Essential Job Functions:
Provides oversight and support for all medical and healthcare related needs of individuals. Conducts training for management and community support staff.
Provides oversight/supervision of Licensed Practical Nurses working in Group Homes and Day Programs.
Provides nursing services to individuals by identifying and reporting on health needs of individuals through observation of their physical, social and emotional needs and through review of their medical records.
Monitors, evaluates and administers nursing care to individuals as needed according to the plan of care.
Contributes to the development, implementation and evaluation of care plans through conferences with the group home manager, case manager, guardian/family and medical professionals.
Attends medical appointments and transports individuals to community destinations as necessary or assigned in Heart to Heart's vehicle. Schedules medical appointments when appropriate.
Exhibits proficiency and implements all tasks needed to service the individuals appropriately from a medical perspective.
Reviews all medical related documents and medication packs/containers to ensure proper administration and documentation.
Administers medication and documents in compliance with Heart to Heart Home Care's policies and procedures which conform to DDD guidelines.
Orients new community support staff to the medical needs of the specific individuals with whom they will be working.
Ensures that policies and procedures that guide and support the provision of care are implemented and the recommendations to improve performance of care are implemented within Heart to Heart.
Reviews and maintains the system of clinical records, reporting, reporting forms and service data, ensuring that all documentation is timely, appropriate, accurate and within agency guidelines.
Performs on-call duties.
