Circle of Care in St. Louis is actively searching for a Registered Nurse (RN) to join their team part-time, dedicating 10-12 hours per week to providing at-home care. This caregiver role encompasses a flexible schedule, allowing you to set your hours!
Our Circle of Care, St. Louis nurses are crucial team members, serving our clients with the utmost compassion and care. Here are the primary duties for this role:
- Providing Maintenance or Preventative Services: Our nurses deliver critical maintenance and preventive care for participants with stable, chronic conditions.
- Medication Management: Nurses support diabetic participants who self-administer their insulin, facilitating pre-filled syringes according to the manufacturer's guidelines. They also set up weekly medication schedules for participants experiencing minor disorientation or confusion.
- Skin Condition Monitoring: Regularly assess and report on the skin condition of participants at an elevated risk of skin complications.
- Health Evaluations: Conducting thorough health assessments routinely helps evaluate the overall health status of the participants.
- Nail Care: Our nurses provide regular nail care services for those with specific health conditions such as diabetes, a compromised immune system, or those on anticoagulant medications.
- On-Site Home Visits: Nurses perform monthly on-site visits for each participant who receives advanced personal care services, thereby continuously evaluating the participant's health.
- Training and Education: Offering real-time training and competency checks for advanced personal care aides ensures the continued delivery of exceptional care.
- Discharge Meetings: Attending discharge meetings at skilled nursing facilities to review medication lists with patients. This essential step ensures patients understand their medication regimen, its purpose, and correct administration.
Ideal Candidate:
- Holds a current RN or LPN license in the State of Missouri. LPN candidates should have at least one year of experience caring for the elderly, individuals with disabilities, or those having complex medical conditions.
- Has a minimum of 2 years of experience in providing care to the elderly, individuals with disabilities, or those having complex medical conditions.
- Capable of conducting home visits and delivering high-quality, person-centered care.
- Ready to provide preventive and maintenance care services not intended to treat acute health issues.
- Experienced in ensuring seamless transitions for clients moving from skilled nursing facilities to a home setting.
- Excellent communication skills and ability to provide culturally sensitive, personalized care to a diverse client base.
- Includes providing in-home nursing services of a preventative or maintenance nature to participants with stable, chronic conditions.
