Registered Nurse Or Licensed Practical Nurse

We are seeking a RN or LPN to join our team!

Responsibilities:

• Create and monitor Risk, Health and Wellness plans/support.
• Monitor and report changes in symptoms or behavior of individual served.
• Communicate with collaborating physicians or specialists regarding the individual's care.
• Educate and train clients, family members and staff.
• Facilitate referrals to other healthcare professionals and medical facilities.
• Maintain accurate medical records and train staff on EMAR.

Qualifications:

• Experience working with individuals with disabilities.
• Ability to build rapport with clients, families and staff.
• Strong problem solving and critical thinking skills.
• Ability to thrive in a fast-paced environment.
