Summary:

The Registered Nurse - Nurse Trainer coordinates health care and medical needs for adults with developmental disabilities at Community Link in order to ensure their best possible health and well-being in accordance with agency’s policies and procedures and according to the Mental Health and Developmental Disabilities Administrative Act, the Illinois Nursing and Advanced Practice Nursing Act and Part 116: Administration of Medication in Community Settings Rule.

The Registered Nurse - Nurse Trainer must embrace the agency’s vision, mission, and core values. Additionally, the Registered Nurse - Nurse Trainer needs to be willing to engage in a person center philosophy and collaborative approach. They need to demonstrate effective communication skills, a professional demeanor, flexibility to changing program needs, and a positive attitude when engaging with participants, families, and staff. The Registered NurseNurse Trainer must have genuine interest in the welfare of participants and their families.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Participate as a full member of the participant’s support network by providing health and medical information important to the wellness of the participants supported.
  • Complete assessments (HRST, SAMA, nursing assessments, TD screenings, etc.) in order to monitor participant health and make recommendations.
  • Provide 24 hour on call nursing support as part of a rotating on call schedule.
  • Addresses various health and medication needs on a daily basis to participants supported including assessments of health condition and implementation of medication changes via EMAR system.
  • Establishes constructive working relationships with participants, co-workers, program managers, guardians/parents or other family members, and other facilities/agencies in order to ensure the health needs of the participants are being met.
  • Reports on the participant’s health and wellness to supervisor, program team, guardians and/or families as required.
  • Ensure agency’s policies and procedures related to general good health and medication administration are communicated and understood by subordinates as well as providing ongoing training and development of staff regarding medical issues, medication administration per Rule 116, and CPR/First Aid; ensuring compliance with the rule and its guidelines including observations and documentation.
  • Delegate, supervise, and authorize the task of medication administration to authorized Direct Care Staff according to Rule 116.
  • Provide professional input as a member of Community Link’s Committees (Human Rights Committee, Behavior Management Committee, etc.)
  • Provide nursing assistance to the Community Day Service program, as appropriate
Requirements:

QUALIFICATION REQUIREMENTS:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.


EDUCATION and/or EXPERIENCE:

RN License; with 2 years of clinical experience within the past 5 years and one year experience working directly with individuals with Developmental Disabilities preferred. Training experience preferred.


LANGUAGE SKILLS:

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.


MATHEMATICAL SKILLS:

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentages.


REASONING ABILITY:

Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.


CERTIFICATES, LICENSES, REGISTRATIONS:

Must have a valid driver’s license and must provide proof of insurance with a minimal of 100,000/300,000 liability coverage. Illinois RN license required. Professional liability insurance recommended.


OTHER SKILLS and ABILITIES:

Effective and efficient communication skills. An ability to adapt favorably to changing program needs. Competent knowledge of behavior principles. Ability or willingness to learn computer operations for client data tracking, incident reporting and other processes deemed necessary by the supervisory staff.


PHYSICAL DEMANDS:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

While performing the duties of this job, the employee is regularly required to use hands to finger, handle, or feel objects, tools, or controls; and is regularly required to talk and hear. The employee frequently is required to stand, walk, and sit. The employee occasionally is required to bend stoop, squat, crawl, climb-height, reach above shoulder level, crouch, kneel, balance and push/pull. The employee continuously is required to lift up to 10 lbs. The employee occasionally is required to lift between 11 lbs – 50 lbs. The job requires the employee to use the left and right hand for repetitive actions for simple grasping, firm grasping and fine manipulating. The employee must have the ability to drive the agency=s van. The employee must be able to provide assistance to individuals as needed such as maneuvering wheelchairs, or assisting clients in ambulating and transferring when necessary.

Specific vision abilities required by this job include close and distance vision.
