AddressSummary The Northport VA Medical Center is hiring one, full-time, permanent RN for the Mental Health Clinic. The MHC is an outpatient program. Influences care outcomes by collaborating with members of the interdisciplinary team The Primary Care Mental Health RN are care managers that collaborates with Primary Health Clinics in providing Mental Health access for same day services for our Veterans. The care manager RN's works closely with Primary Care Mental Health Integration team in providing clinical oversight of the program. The Primary Care Mental Health Integration Care Manager is responsible for monitoring Veterans to ensure they remain connected with the appropriate services. The nurses provide same day triaging, referrals, medication monitoring, and referral management. Additionally, the care managers review a list of Veterans who have been inconsistent in their Mental Health follow-up appointments and offers to reconnect them with available services. The Staff RN is also responsible and accountable for maintaining training required and may be assigned additional duties as area needs require. The Staff RN is expected to communicate timely and effectively with all members of the patient care team, as well as relate courteously and appropriately to all internal and external customers. The Staff RN must be able to push-pull and lift 35 pounds and must be able to stand for prolonged periods of time. Finally, the Staff RN is expected to work in a collaborative, team-oriented environment, and demonstrate consistent effective communication skills and exemplary professional behaviors. Physical Requirements: The position requires the incumbent to have visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires limited periods of standing, stooping, bending, pulling, pushing, walking, and be able to lift up to 35 pounds. Transferring patients and objects may be required. This position also requires sitting for long periods typing on computer keyboard, looking at computer monitor, and using the telephone. The incumbent maybe exposed to infected patients and contaminated materials requiring the incumbent to don protective clothing and equipment in isolation situations or during invasive patient procedures. The incumbent may be exposed to patients who are combative or aggressive related to neurologic or psychiatric disorders.
