Key Responsibilities:
- Patient Assessment and Care Planning:
- Conduct comprehensive assessments of patients with medical/surgical conditions, including monitoring vital signs and evaluating responses to treatments.
- Develop and implement individualized care plans based on the patient's medical history, diagnosis, and healthcare team input.
- Coordinate with physicians and other healthcare professionals to optimize patient outcomes.
- Medication Management:
- Administer medications and treatments as prescribed, ensuring accuracy and adherence to established protocols.
- Monitor for potential side effects and complications, taking prompt action to address any issues.
- Educate patients on medications, potential side effects, and the importance of compliance.
- Wound Care and Surgical Support:
- Provide skilled nursing care for patients recovering from surgery, including wound care, monitoring postoperative conditions, and managing pain.
- Collaborate with the surgical team to ensure a smooth transition from the operating room to the postoperative care unit.
- Educate patients and their families on postoperative care and recovery expectations.
- Patient and Family Education:
- Educate patients and their families about their medical conditions, treatment plans, and self-care measures.
- Provide guidance on lifestyle modifications, dietary considerations, and follow-up care.
- Foster open communication, addressing questions and concerns to enhance patient understanding and satisfaction.
- Collaboration with Healthcare Team:
- Work closely with physicians, nurse practitioners, and other healthcare professionals to coordinate and deliver comprehensive patient care.
- Participate in interdisciplinary rounds, contributing insights and ensuring effective communication.
- Serve as an advocate for patients, ensuring their needs are met and preferences are considered.
- Documentation and Compliance:
- Maintain accurate and up-to-date patient records, documenting assessments, interventions, and outcomes.
- Adhere to regulatory standards, institutional policies, and quality assurance measures.
- Contribute to continuous quality improvement initiatives and participate in performance improvement activities.
