ABOUT THE POSITION

The County of Riverside - Riverside University Health System- Behavioral Health is currently recruiting for Registered Nurse II/III/IV for our Housing Regional Administration Team located in Riverside, Ca. The Registered Nurse will be responsible for the following:

  • The RNCM supports ECM Members with complex medical conditions and completes medication reconciliation in collaborations with pharmacy, when possible, for all ECM enrolled members.
  • Responsible for working with a caseload of Members with complex medical needs; Conduct face to face visits as determined by the Member's tier.
  • Engages Members and supports/encourages active participation towards achievement of their health goals.
  • Tracks medical and Behavioral Health outcome measures in the web-based care management platform.
  • Provides Members and family education about chronic medical and Behavioral Health conditions to improve health literacy.
  • Facilitates and ensures recommendations are communicated across the health Providers.
  • Works with Members to identify health/wellness goals and incorporates these goals into Health Action Plans/Shared Care Plan that facilitate communication among Members and Providers.
  • Champions healthy lifestyle changes.
  • Coordinates physical care management and care coordination relationships with external healthcare Providers.
  • Collaborates on Member care issues with other team members and participating in Systematic Case Reviews (SCR) and ad hoc case reviews.
  • Receives, identifies and follows up on treatment plan and medication alerts.
  • Consults with the ECM Providers about clinical concerns or questions, and provides educational training on chronic disease states, prevention, treatment, medications and healthy living.
  • Conducts medication reconciliations with input from the Member's PCP.
  • Reviews Comprehensive Health Assessments (CHA) upon completion by other team member; a split role with Behavioral Health Care Manager (BHCM)
  • Ensures smooth transitions of care, and coordination with hospitals and facilities regarding Member admission/ discharges.
  • Tracks and assures required assessments and screenings are performed, including Comprehensive Health Assessment and Shared Care Plan.
  • Responsible for promoting a collaborative and effective working environment within the ECM by engaging in evidenced-based communication strategies (such as Motivational Interviewing) when discussing responsibility/sharing of tasks, resolving conflicts as they arise, and collaborating on Member case discussions.
  • Responsible for ongoing engagement with Members, both in-person and on the phone, in a manner that draws on evidence-based approaches to promotes collaboration between the Member and the medical/behavioral team, as well as to increase the Member's sense of control over their whole health.
  • Provides consultation for all members of the team when discussing tasks of a clinical nature.

Work Schedule: 9/80, 8:00am-5:30pm Every other Friday off. This is afield-basedposition(Youwill be seeing consumers in their homes or in the community)

Required Certifications and Licenses:

  • Must possess and maintain a current valid license to practice as a Registered Nurse in the State of California.
  • Possession of valid Basic Life Support (BLS) certificates issued by the American Heart Association.
  • Possession of a valid California Driver's License may be required.

EXAMPLES OF ESSENTIAL DUTIES
• Within the full scope of the Nurse Practice Act, assess, plan, organize, and provide nursing care to assigned patients in accordance with physician instructions; make preliminary observations of, and prepare patients for, medical treatment.
• Assist physicians in administering treatments and in performing medical examinations, diagnostic tests, and surgical operations.
• As directed, administer or dispense prescribed treatment and medications to patients; chart treatment and dispensing to patients, and record and report significant changes in condition and general progress of patient.
• Counsel and advise patients, families and significant others regarding special medical problems and/or proper healthcare methods.
• Prepare and maintain concise and complete records and reports; instruct patients in carrying out physicians' orders; may transcribe physicians' orders to working records.
• May provide orientation and training to new staff, residents, medical students, ROP and nursing students.
• May participate in quality assurance reviews; may act as a team leader or relief charge nurse.
• Assign and monitor the work of subordinate nursing personnel; participate in committee activities; identify staff training needs.
• May triage patients by reviewing admissions charts and merging patients into the clinic flow at the appropriate time and place.
• May testify in court as to the client's mental and physical condition; may recommend initiatingconservatorshipinvestigations when assigned to the mental health setting.
• May coordinate and monitor patients through the hospitalization process utilizing physicians, primary care nurses, and third-party payors to maintain quality care and fiscal responsibility.
• May review the post hospital care plan with the patient/family, establishing a contact regarding timeframes and responsibilities; follow plan through a discharge.
MINIMUM QUALIFICATIONS
Registered Nurse II ($39.25-$46.95/hr)
Education: Successful completion of the education required to obtain a valid license to practice as a Registered Nurse in the State of California. (A bachelor's degree from an accredited college or university in nursing or a health-related field may be required for some positions.)
Experience: Completion of one year as a Registered Nurse in an acute care hospital, mental health setting, clinic outpatient setting, or healthcare agency program. Experience gained as an Interim Permit Nurse may be credited toward meeting the required experience. Evaluations and references must show proficiency of practice as a Registered Nurse.
Education: Graduation from an accredited college or university with a bachelor's degree in nursing or health-related field or current and valid national certification in nursing or health-related field.

Registered Nurse III ($42.17-$53.67/hr)
Education: Successful completion of the education required to obtain a valid license to practice as a Registered Nurse in the State of California.
Experience: Three years of full time experience as a Registered Nurse in an acute care facility, mental health facility, or community health agency with references and evaluations to show appropriate skill level. (A bachelor's degree from an accredited college or university in nursing or national certification in a nursing specialty may be considered as a substitution for one year of the required experience. A master's degree from an accredited college or university in nursing may be considered as a substitution for two years of the required experience.)

Registered Nurse IV ($45.12-$56.62/hr)
Experience: Six years as a full-time Registered Nurse. Some positions may require two years in a specialty area. A master's degree from an accredited college or university in nursing or a health-related field may be substituted for two years of the required experience.

Knowledge of: Current principles, techniques and procedures used in professional nursing; medical terminology and patient care routine appropriate to clinical setting; medical instruments and equipment; the uses and effects of medicines and narcotics; community resources available to provide patient care and follow-up; federal, state and local laws and regulations governing professional aspects of nursing; identification of potential discharge needs; payor source documentation requirements and governmental regulations affecting reimbursement.

Additional Working Knowledge Requirements for Mental Health Applicants: Current child, dependent adult, and elder abuse reporting laws; current involuntary detention and commitment laws related to mental health; involuntary psychotropic medication laws; andTarasoff-dutyto warn requirements.

Ability to: Perform the full range of nursing duties and responsibilities in a patient care setting; plan, organize and implement nursing plans for patients; prepare and maintain concise and complete records and reports; establish and maintain effective working relationships with physicians, patients and co-workers; secure the cooperation of employees to work effectively together; keep abreast of developments in areas of nursing specialization; teaches, mentors and supervises others routinely; effectively prepare and present reports and recommendations both orally and in written form; evaluate the medical records of hospital admissions; provide recommendations regarding continued stay necessity; identify discharge needs; collect and evaluate data, draw conclusions and formulate recommendations.

In Mental Health, ability to use individual professionally sound judgment to make complex decisions related to: triage; clinical assessments including biopsychosocial evaluation; integration of mental health, chemical dependency, and physical healthcare needs; potential suicidality; placement issues and necessary linkages and supports; and need for emergent psychiatric care and consultation.

Other Requirements
License/Certificate: Must possess and maintain a current valid license to practice as a Registered Nurse in the State of California.

Possession of valid Basic Life Support (BLS) certificates issued by the American Heart Association.

Possession of a valid California Driver's License is required.

Pre-Employment
