Responsibilties:

We are looking for compassionate, detail-oriented RNs to plan and organize hospice care for our patients in their homes and facilities.

• Develops the Plan of Care in conjunction with the patient, physician and interdisciplinary team to assume appropriate utilization of services and optimal client outcomes.
• Serves as case manager and assumes responsibility to coordinate patient care to maintain the highest level of care for comfort and quality of life as a goal.

• Attends IDT meetings to review hospice patient care plans.

• Participates in on-call rotation and provided appropriate patient's support.
• Applies nursing skills and techniques in the treatment of patients, providing emotional support and education to patients and families.

Qualifications:

• License as a Registered Nurse in PA
• One Year Hospice/Homecare Experience Preferred, but not required
• Valid Driver's License
• Excellent Communication and Critical Thinking Skills
• Understanding of Hospice Philosophy, Principles of End-of-Life Care
• Self-Directed with the Ability to Work within a Team Setting
• Criminal Background Check Clearance

Job Type: Full-time

Pay: $72,000.00 - $77,000.00 per year
