ADVANCED HOSPICE IS GROWING, and we are actively seeking a compassionate and dedicated Hospice On-Call Registered Nurse to cover our Beaver and Allegheny County patients!
As an On-Call RN, you will work a rotating schedule to provide nursing care and support, outside of regular hours, to patients where they reside, including private homes and assisted living facilities. This position will be after-hours on-call working seven days/nights on and seven nights/days off. Hours will consist of: 4 PM to 9 AM shift Monday-Friday; Friday 3 PM to Monday 9 AM. You may sleep during your shift, however you MUST wake up to incoming phone calls to address patient needs.
This role will consist of answering after hours calls, triaging patient needs, seeing declining patients, completing evening admissions, and completing chart audits.

Key Responsibilities
- Respond promptly to after-hours calls, providing skilled nursing care and emotional support to patients and families facing end-of-life issues.
- Current Registered Nurse (RN) licensure (in good standing) in Pennsylvania with one year of experience under license
- Oversee the implementation and ongoing assessment of the patient’s plan of care
- Work closely with the IDG team (spiritual care coordinator, social worker, hospice aide and medical director) to ensure proper coordination and needs for patients and families.
- Works well with patients on a 1-1 level with strong patient care team support.
- Ensure an accurate evaluation and work to develop and implement a comprehensive care plan that addresses patient needs.
- Communicate patient conditions and collaborate with appropriate providers to deliver care when patient needs evolve.
- Admissions and On-call experience preferred.
- Must be willing to drive with reliable transportation, a valid driver’s license, and auto insurance, to be able to travel across the service area.

Requirements
- Licensure: Current license to practice as a Registered Nurse (RN) in the State of Pa.
- Current CPR/FA certification.
- Valid state-issued driver’s license, automobile insurance, and reliable transportation; willingness to travel.
- One (1) year of professional experience practicing as a Registered Nurse (RN) in a hospice or similar setting.
Advanced is willing to provide Employee with three (3) options in relation to a Sign-On/Retention Bonus. Those being a one year, two year and three-year option. Advanced agrees to pay Employee a Sign-On/Retention Bonus of up to a period of three (3) years, subject to the Repayment provision
