JOB DESCRIPTION
Work Schedule: 0.5 FTE: Every other Thursdays 8pm-8am, Fridays 4:30pm-8am, Saturdays 8am-8pm.
Thursday: 430pm – Friday 0800
Friday: 430pm – Saturday 0800
Saturday: 8pm- Sunday 8AM
Required Qualifications:
- Associate’s nursing degree from an accredited school
- Must have MN or WI RN license upon start date.
- Must provide proof of MN and WI RN license within 4 months of start, if both MN and WI RN licenses are not present on start date.
- Must have a dependable vehicle, valid driver’s license, auto insurance and meet all criteria required under Lakeview’s Automobile Liability Guidelines
- Knowledge of reimbursement procedures as well as knowledge of community resources
- Accept responsibility for maintaining current knowledge level of nursing trends in the Hospice field.
- Able to function independently with minimal supervision in home setting
- Strong assessment skills
- Ability to read, write and speak the English language
- Ability to carry out directions
- Computer experience
- Comfortable in end of life care
- Flexible with changing schedule
- Willing to participate in On-Call rotating schedule
Preferred Qualifications:
- One year experience of Homecare, Hospice, or Long-Term Care
