Optimal Hospice is looking for a passionate and seasoned Registered Nurse Case Manager (RNCM) to join the team. The Registered Nurse will provide care to patients at their home or placement. Mileage will be reimbursed.

THE OPTIMAL HOSPICE REGISTERED NURSE:

• Hospice heart: compassionate, kind and empathetic to your patients and their families
• Nurse/Patient Relationship: Develops and maintains a therapeutic relationship through the health care continuum
• Provide direct patient care services to hospice patients
• Empower patient's caregivers by education them on care for their loved one
• Provide coordination of care, ensure continuous assessment of each patient's and family's needs
• Responsible for documentation and communication of care, medication, and observations made during care of the patient in order to track responsiveness and health of patient.
• Responsible for providing appropriate patient/family education to help patient/family achieve a pain-free and comfortable status.

Pay: $88,000-100,000 DOE

REQUIRED QUALIFICATIONS:

• Current Colorado RN license
• Experience as a RN
• Current CPR
• Possess a valid state driver license
• Ability to lift up to 35 pounds without assistance.
