Seeking a Registered Nurse- Home Visit Care Coordinator for our Clinical Excellence Team.

PRIMARY RESPONSIBILITIES:
- Provides nursing services in patients’ homes (IV administration of approved medications and therapies, other licensure-approved and SOFHA-approved treatments) to SOFHA HOME VISIT patients under the order and direction of the HOME VISIT team providers and their supervising physician.
- Uses SOFHA-approved triage protocols and nursing judgment to prioritize urgent HOME VISIT patient appointments.
- Answers incoming calls/messages from patients and referring SOFHA providers regarding HOME VISIT appointments.
- Works in conjunction with other HOME VISIT team staff to ensure quality patient care.
- Participates in the transition of patients from the inpatient setting to home or to a post-acute setting with a goal of reducing readmission risk and improving outcomes.
- Identifies and refers patients to services which may benefit the patient’s achievement of health goals.
REQUIREMENTS:
- Graduate of an approved school of nursing as an RN
EXPERIENCE:
- Minimum of two years’ experience, preference given to those with home health or ED/ICU experience. Must be comfortable with initiation of IV lines and administration of IV medications.
CERTIFICATES, LICENSES, REGISTRATIONS:
- Valid license to practice in the state of employment and current CPR certification required. IV Certification is desired.
