- Provide skilled nursing care to patients in their homes
- Conduct comprehensive assessments of patients' health status
- Develop and implement individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor and evaluate patients' progress and make necessary adjustments to the care plan
- Educate patients and their families on disease management and self-care techniques
- Collaborate with other healthcare professionals to coordinate patient care
- Document all patient interactions and interventions accurately and timely
```Requirements```
- Valid Registered Nurse (RN) license in the state of [location]
- Minimum of [X] years of experience as a Registered Nurse, preferably in Home Health or a related field
- Proficient in ostomy care, pain management, dementia care, esthetic laser treatment, Cerner, ICD-9, ambulatory surgery center procedures, IV insertion, and inhalation therapies
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Compassionate and empathetic attitude towards patients and their families
