Expectation/Requirements:
- Primary focus on patient care.
- Accurate, timely documentation.
- Auto insurance – Current.
- Case Management experience required.
- Conflict resolution skills.
- CPR certified.
- Critical thinking skills.
- Driver’s license – Current and unrestricted.
- Follow current plan of care.
- Motivated self-starter with an approachable, friendly, and positive attitude.
- OASIS Initial Assessments.
- Observation and Assessment skills.
- Paid on-call rotation.
- PDGM – Must have worked in this newest payment model.
- Professional license – Current and unrestricted.
- Skilled Home Health Care – 1-2 years of clinical experience required.
- Strong communication, organizational, and time management skills.
- Strong provider communication.
- Team leads provide supervision of assistants (LPN, HHA).
- Transportation – dependable and reliable.
- Willingness to learn and help support other team members.
