We’re Looking for Registered Nurses Who:

• Will provide the best patient care around
• Love working independently, but want to have a team who is always there to support them when needed.
• Are great communicators, both written and orally.
• Want to work part-time or fulltime in home health in the Green Bay, and surrounding areas.

Requirements: 

• Currently licensed in Wisconsin as a Registered Nurse.
• Current American Heart Association CPR Certification.

 Job Duties include:

• Acts as Case Manager and assumes responsibility to coordinate patient care
• Complete and document a thorough assessment of patient and develop individualized patient treatment plans
• Work with clinical staff, patient and families to maximize patient outcomes
• Document the course of treatment in a clear, concise manner, including a discharge summary utilizing the prescribed format.
• Instruct Licensed Practical Nurses in treatment procedures to be utilized and provide supervision
• Collaborate with clinical team to develop and educate patient and/or care givers on safe discharge plan
