STATEMENT OF PURPOSE
Provides direct medical and programmatic nursing care to patients utilizing the nursing process and participates in patient and family teaching and discharge planning. Assumes responsibility for completing assigned duties and for supervision of non-licensed nursing staff, LVN’s, and LPT’s. Works cooperatively with patient team personnel to maintain standards of professional nursing practice in the clinical setting. ___________________________________________________________________________________________________________
QUALIFICATIONS
Current license to practice professional nursing in the State of California. Ability to assess the age specific needs of the patients per unit population. BCLS required. Management Assaultive Behavior (MAB) is required within 90 days of hire. __________________________________________________________________________________________________________
PHYSICAL DEMANDS
Occasional sitting for long periods of time. Frequent walking, standing, bending and reaching. Occasional lifting with assistance up to 250 pounds. Frequent pulling and pushing up to 100 pounds. Corrected vision and hearing to a normal range. Finger dexterity. Constant exposure to stressful situation. WORKING CONDITIONS
Frequent exposure to noise. Occasional exposure to dirt, dust, smoke and fumes. Occasional exposure to cold, heat, chemicals and general cleaning chemicals. Rare exposure to bloodborne pathogens. Demonstrates ability to teach patients/families regarding psychotropic medications and identified medical problems
Demonstrates understanding of obtaining medication consents.
Demonstrates ability to direct Emergency Codes., remaining calm
Verbalizes understanding of CSU’s best practice on psychiatric homeless patient discharge planning including ensuring they have proper clothing, footwear and are offered food
Demonstrate an understanding of the admission and discharge process and paperwork involved
Demonstrates the ability to assess medical conditions and respond appropriately with any abnormal findings reporting to the charge RN
Demonstrates ability to assess patients for withdrawal symptoms of substance abuse.
Demonstrates the ability to recognize and intervene when clients are decompensating
Demonstrates the ability to ensure safety on the unit at all times and notify Charge RN or manager when an unsafe situation occurs.
Demonstrates effective team building competency by working with other team members during heavy workload and by assisting others when needed.
Demonstrates ability to follow all fire and safety guidelines and knows role in case of emergency/disaster
Demonstrates knowledge of Infection Control, Standard Precautions, and protecting against
blood-borne pathogens, isolation technique, PPE and red bagging, proper use of mask to prevent the spread of airborne pathogens
Demonstrates and consistently practices proper handwashing techniques in order to help control the spread of infection
Demonstrates excellent Customer Service skills and sensitivity to cultural diversity when dealing with co-workers, patients, physicians and outside agencies.
Demonstrates competent, safe and effective equipment used on the unit and protocol for removing unsafe equipment
Demonstrates the ability to perform chart reviews and break down charts after a client discharges
Demonstrates sound knowledge base and actions in the care and decision making for severely mentally ill patients.
Performs assessment/data collection in an ongoing and systematic manner, focusing on physiologic, psychological and cognitive status.
Formulates a goal directed plan of care which is prioritized and based on determined nursing diagnosis and patient outcomes.
Educates patient on learning needs according to care plans.
Performs documentation duties on unit and in patient record which are timely, accurate and concise.
Demonstrates an awareness of self-responsibility and accountability for own professional practice.
Remains flexible in responding to unit needs regarding schedules and resolution of staffing.
Demonstrates appropriate delegation of tasks and duties in the direction and coordination of the work of nursing team members, patient care and unit activities.
Demonstrates effective communication methods and skills, using lines of authority appropriately and keeps self informed of content of staff meetings, new policies and other significant information.
Demonstrates awareness of and sensitivity to patient rights, as identified within the institution and by the California Department of Mental Health.
Performs efficiently in emergency patient situations following established protocols, remaining calm, informing appropriate persons and documenting events.
Practices effective problems identification and resolution skills and report identified issues to Charge RN.
Assesses age specific needs and communicates with patients on assigned units in language that patient can understand according to their age.
Continuously recognizes and utilizes opportunities for teaching LVN/LPT & MHW staff relative to their work with patients on assigned units.
Holds staff accountable for safely monitoring patients and reporting any unsafe occurrences.
As the coordinator of care, the Registered Nurse shall be responsible for the observation and direction of patient lifts and mobilization, and shall participate as needed in patient handling in accordance with the nurse’s job description and professional judgment.
Performs other duties as assigned.
