Hiring for a Emergency Room RN at Carl R. Darnall Army Medical Center in Fort Hood, TX. $10,000 Sign-On Bonus!

Desired Qualifications:

Degree/Education: Associates Degree in Nursing (ADN) or Bachelors of Science in Nursing (BSN). BSN is preferred but not required. Diploma in Nursing or Associates Degree is required. Graduate from a college or university accredited by National League for Nursing Accrediting Commission (NLNAC), or The Commission on Collegiate Nursing Education (CCNE).

Certifications in addition to Basic Life Support: In addition to BCLS/BLS, certification in Advanced Cardiac Life Support (ACLS)/Advanced Life Support (ALS) and Pediatric Advanced Life Support (PALS) is required for the ER. BCLS/BLS, ACLS/ALC, and PALS certification must be obtained through an American Heart Association (AHA) or American Red Cross (ARC) training program. Trauma Nurse Core Course (TNCC) must be obtained and passed within six (6) months after initial start date. Certification in Emergency Nursing is highly preferred. Certification in Emergency Nursing and/or Critical Care Nursing is highly preferred.

Experience: A minimum of 1 year experience as a practicing RN within the last 3-years. Six months out of that 1-year experience must have been in a Level III Emergency Room or in a critical care area with four (4) or more beds.

REQUIREMENTS:

Must possess evidence of continuing nursing education that contains knowledge and skills required in the specific nursing specialty.

Must pass an ER specific competency based orientation and assessment, within the first 5-shifts, that encompasses requirements unique to the particular specialty of an ER RN. Assessment test (Exhibit A) will be administered within the ER during patient care and may consist of recognition of life threatening arrhythmias; defibrillation; emergency cardioversion; independent administration of emergency drugs (monitor, mix, administer, titrate); ability to operate Belmont rapid infuser, administer whole blood, PRBC, and knowledge of mass transfusion protocol, treatment of post-partum hypertension/pre-eclampsia patients and industry standard roles of a trauma nurse.

Must possess professional knowledge in a wide range of nursing concepts, principles IAW Standards of Nursing Practice accepted at national level.

Must have knowledge and ability to evaluate situations involving all administrative and professional areas for outpatient care to determine alternative approaches for care and services.

Must have knowledge of health care management principles and techniques to recognize needs for process improvement and offer solutions to a wide variety of management problems.

Must have knowledge of counseling, evaluation, and assessment techniques as well as educating patients on general healthcare topics.

Knowledge and ability to communicate effectively with healthcare providers, patients and supporting community agency staffs on general and specific requirements for treatment of disabled military patients.

Must maintain alertness, vigilance and skill in reacting promptly to emergencies such as treatment of trauma, drug overdose, psychosis, cardiac events, respiratory compromise, and emergency childbirth.

Knowledge of anesthetic agents and pharmaceuticals, their desired effects, side effects, compatibility with other drugs and complications of their use.

Administers moderate sedation and implements appropriate monitoring post-procedure to all ages of patients. Requires certification in the use of moderate sedation agents and monitoring of all ages during recovery phase of moderate sedation and monitoring of deeper sedation patients on mechanical ventilation IAW hospital policies and procedures.

Requires knowledge of pharmacological and other paralytic agents and monitoring required to perform rapid sequence induction in order to assist with emergency intubations of respiratory compromised patients.

Skill in operation of specialized medical equipment. Monitors and maintains central lines (arterial and central venous pressure), chest tubes and other invasive cardiac/thoracic monitoring procedures.

Knowledge of a wide variety of medical disorders, injuries and surgical interventions. Anticipates complications and indicated therapeutic interventions for all ages of patients.

Knowledge of sterile and aseptic technique.

Skill in written and verbal communication sufficient to obtain, disseminate and/or document patient care information using manual means or computer driven patient information systems.

Demonstrated knowledge and skills to provide care appropriate to the age of the patient served on assigned unit. Must demonstrate knowledge of the principles of growth and development over the life span. Must be able to assess and interpret data about the patient's status to identify each patients' age specific needs and provide the care needed by the pediatric, adolescent, adult, and/or geriatric patient group.

Core Duties:

Coordinate patient care in collaboration with a wide array of healthcare professionals. Facilitate the achievement of optimal outcomes in relation to clinical care, quality and cost effectiveness.

Ensure compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the MTF.

Perform physical exam and health histories.

Provide health promotions, counseling, and education.

Administer medications, wound care, and numerous other personalized interventions.

Direct and supervise care provide by other healthcare professionals.

Accountable for making patient care assignments based on the scope of practice and skill level of assigned personnel.

Recognize adverse signs and symptoms and quickly react in emergencies.

Communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family and organization.

Make referral appointments and arrange specialty care as appropriate.

Perform nursing services identified in the TO.

Conduct research in support of improved practice and patient outcomes.

Perform assigned duties with regard to all patients regardless of underlying process or physical condition.

Performs nursing triage within the ambulatory care arena. Makes independent triage decisions based on assessment and management skills for all ambulatory patients seen in the Emergency Department. Utilizes physician driven protocols to deliver care. Screens patient health history, complaints, medications, and identify health related conditions that may impact the patient’s health status.

Screens patient lab data and notify physician of abnormal results. Performs specific point of use testing which may consist of urinalysis, urine HCG, glucometer specimens, and bedside iSTAT testing, documents results, and performs quarterly testing. Ensures authorization for procedures has been appropriately completed and documented. Monitors patients or directs monitoring of patients during and after procedures.

Functions within the Scope of Practice for a professional Registered Nurse, hospital regulations, and policies. Coordinates patient care activities. Manages clinic and patient records. Follows quality improvement and internal control activities for the ambulatory setting to include risk appraisals, chart audits, trend identification and reporting corrective actions. Receives, investigates, and documents on patient complaints pertaining to their shift in the Emergency Department.

Independently instructs and demonstrates to patient/family/significant others how to provide self-care to include wound care, changing dressings, and catheter care. Instruct patients in self-medications of prescribed pharmaceutical agents. Reinforces physician instructions and encourages patient in maintenance of health and self-care.

React appropriately to de-escalate situations within the standards of Prevention and Management of Disruptive Behavior (PMDB) while relating to patients and families to manage distraught, irritable, unreasonable or angry individuals and to maintain a positive rapport with all individuals involved in patient care.

React with alertness and skill in any emergency, e.g., behavioral health, cardiac or respiratory arrest, hemorrhage, shock, severe physical trauma and psychiatric reaction. Serves as member of resuscitation team.

Assess patients' conditions for potential or life threatening crises. Distinguish between normal and abnormal physical findings (from physical assessment and vital sign assessment). Plan appropriate nursing care.

Obtain patient history and pertinent family history.

Document plan of care and care given in patient's clinical record

Utilize appropriate nursing personnel to assist in delivery of care when indicated (LPNs/paraprofessionals).

Transcribe or verify transcription of physician orders.
